I would like to take this opportunity to welcome you to my practice. I look forward to meeting you and providing the highest quality care. In order to facilitate your appointment, we ask that you please take a few moments and complete the registration and medical history forms listed to the left. Please bring the completed forms with you to your first appointment. By doing this, we hope to make your visit as enjoyable as possible.

 

The following is a short list of items for your information:

 

1)     The first appointment will take approximately one hour for the consultation and exam.

 

2)     Please request all pertinent past medical records from your previous/current physician. A release of information is to the left. After completing the form, RETURN IT TO YOUR PHYSICIAN so they can forward your records to us.

 

3)     Please bring a list of current medications or your pill bottles.

 

4)     If you are diabetic, please bring your blood sugar logs/readings with you.

 

5)     In the event that you cannot keep your appointment, please give us at least 24 hours notice.
